Title: Family Caregiver Project Coordinator
Location: Chicago, IL
Job Type: Contract (8+ Months)
Shift: Monday – Friday 8:30am – 4:30pm and evenings / weekends as needed
Mode: Onsite
Primary Responsibilities:
• Establishing awareness of Caregiver Portfolio, Caregiver Education, dementia care services and disseminating caregiver information
• Presenting trainings and workshops on caregiving and dementia as well as the coordination of outreach for Memory Cafes run by delegate agencies.
• Supporting presentations and programming at the Regional and Satellite Senior Centers and community partners such as aldermanic offices, hospitals, and libraries.
• Providing one to one caregiver support and caregiver program recruitment.
• Administering TCARE screens and assessments and providing SHIP information and Medicare / Medicaid counseling.
• Entering data information into Salesforce
• Community outreach
• Other duties as assigned
Qualifications:
• Graduation from an accredited college or university with a Bachelor’s degree or higher in Gerontology, Psychology, Social Services, Public Health or a directly related field preferred; or at least two years of work experience in the planning, implementation, and administration of social service programs and / or lived experience as a family caregiver.
• Previous experience an emphasis on Senior Services or the aging population and or family caregivers
• Knowledge of social services programs and resources
• Knowledge of resources and services provided by community social service agencies
• Active Listening

• Communication Skills (Oral, Written)
• Organizational skills
